Although every application is different, the following is for a typical ventilation bath fan.
Remove the grille (cover).
Unplug the motor.  The motor is attached to a bracket.
Locate and remove the screw that hold the bracket in the housing. 
Take out the complete bracket with the motor and blade/wheel still attached and try to locate the motor number.  If there are no numbers on the motor, look back up in the housing for a label that will have the model number of the fan on it.
It there is no label, bring the whole assembly to us for identification, or take some photos and email them to us.  Make sure the pictures show the bracket, and blade.
